Assistant Grep never matches the line numbers it's expecting

I’ve seen this basically every time it’s grepped something, happened with claude API key as well.

Expected behavior

I’d expect the line numbers to match, something is going wrong somewhere.

I swear I’m not editing anything, it’s just getting incorrect results from its grep tooling.

Yeah, it seems like Grep is just generally in a fail-state right now. I don’t think I’ve seen a single prompt in the last few days that wasn’t struggling with Grep line numbers and falling back to manually crawling.

Some prompts even end up writing editor scripts in-line to fetch the script contents they need. Wild.

I am seeing it fire out an occasional internal service error during grep calls while it identifies the file I think. It’s occasional and fixes after a continue. Maybe the alt-path is being overwhelmed with requests? I’m not in your code, this could be unrelated P:
