Teleport Failed: Unable to confirm safety of teleportation place

Recently (within the past day or 2), users have reported seeing a message saying unexpected error (image of console attached). It is teleporting to the same placeid. Wondering if engineers are aware of this, or if anyone else is having this occur. It doesn’t happen 100% of the time.
