Unknown error, Please try again [Maturity Compliance Questionairre]

Yesterday my content maturity rating got denied, so i had to redo the form. However, everytime i load into the form page,
this error appears.

And then every time i try submit the form,

This error appears.
So it’s an endless loop, and the worst thing about this is that my game is now taken down. It’s been down for around 15 hours now and it’s really hurting the game.

I request that this be fixed with urgency.

Hi, apologies that we did not see this as urgently as needed. Can you confirm if this is still happening? We submitted a couple of fixes over the past month.
