What are some considerations that need to be taken when planning a large scale game?

I am heavily considering starting a second project after learning so much from my first one. I’d love a large scale medieval game with a nice terrain map and such. There wouldn’t be a game script, so nothing that is constantly controlling the “flow” of the game, but I’d like to of course have the map be as “alive” as possible. For example, animations, animals, NPCs, etc. My biggest concern though would be optimization. I want the map to be large. What are some tips or tricks that you could tell me before starting such a large project? What are some things that you wish you knew before starting yours?
