Typically as part of a workflow, at some point you would decimate, then retopologize the sculpted object. Then you can hand preserve the most important details by increasing density in those areas, while smoothing over more details in other areas to keep under budget.
I figured the topology would need cleaning. Like how when you 3D scan something, there are thousands of little bumps and creases picked up.
