Information
This thread contains a bug sheet that can be useful when having your game tested, this bug sheet can be used by both bug testers/hunters and developers.
The bug sheet contains the following sections:

# - This is the section that numbers all the bugs, there are a total of 1000 rows for bugs.

Issue/Bug Description - This is the section where the tester/bug tester provides detailed information about the bug and how the bug occurs.

Status - This section contains a drop-down list that has 6 different values to choose from, these will be described in more detail further into this thread.

Where was the bug found - The testers/bug hunters provide the location in which they have found the bug, it could be somewhere on the map, it could be a model and it could even be the developer console.

Proof of the bug - This is the section where the tester/bug hunter provides screenshot proof of the bug, it can also be used to provide the information that the developer console displays.

Type of bug - The testers/bug hunters use this section to provide the type of bug that they encountered, whether it is a script bug, model bug, or UI bug or anything else.
Status

Low - This value is chosen if the risk that the bug brings is relatively low, for example, a slight miss-placement would be considered low.

Medium - This value is chosen if the risk that the bug brings, somewhat affects the gameplay, for example, a small script that doesn’t work would be considered medium.

High - This value is chosen if the risk that the bug brings, affects the game pretty badly and could affect the gameplay, for example, a script that is supposed to do something to the player doesn’t do anything.

Critical - This value is chosen if the risk that the bug brings, is extremely important and makes the game unplayable, for example, a script that doesn’t let the play proceed to the next part of the game.

Suggestion - This value would be used when the tester/bug hunter would like to give their ideas on what the developers could add to the game.

Solved - This value would only be used by developers when the bug/issue that occurred is solved.
How to download
In order to get a copy of the bug sheet, you will need to follow a few steps that will be demonstrated with the images below.
Step 1
Click HERE to open the bug sheet using google sheets. When you have done that, your screen should look like this:
Step 2
Once you have opened the bug sheet, you will need to create a copy of the document, you can create it by clicking File. After you’ve done that, you should see a drop down list that says Make a copy.
Step 3
Once you’ve created a copy, you should get a pop up that tells you to rename it, this is where you replace {PROJECT NAME} with whatever your game or project is called.
