Bugged item "The Pain Bringer"

This item works in r15 when equipped like normal, however when its equipped in r6 mode this item does not function at all and it instead goes through the characters arm instead of it being held like it should as shown in r15.

I have provided images of the issue working in r15 mode and images of it being used in r6 mode which is completely bugged and instead of it being held it goes through the players arm.

Expected behavior

I would like to see this item get fixed for the r6 mode.
