I’d make sure you add a cooldown on the submit button otherwise people can spam it, causing you to potentially be rate limited (if you dont have a cooldown already). Otherwise it looks great.

it looks nice, to improve it you could add few things:
from roblox side:
-cooldown (like a person before me mentioned)
-force player to select reported person, so players cant do reports on nobody
from discord side:
-show name of player who sent report
-show name of reported player
making these changes will improve usefulness of this bot for sure

on top of these, add a description box too
and maybe a disclaimer saying bans can be served for false reports
