If you join a game with gear enabled, then one of your gears can auto equip. If it does that, it will soon fall through the floor into the void and be removed from your inventory. This only happens on join. You can easily recreate this in studio using the city template.
To recreate: Equip a gear on your avatar. Join a game with gear enabled. Watch yourself lose that gear.
