How do I round out the hands like this??

You could insert a sphere, give it the same skin color and attach it to the arm.
Although it’s recommended that you use R15 or another realistic package if you want hands.

You can use a subdivision surface modifier or use a smoothing deformer set to smooth. Though smoothing is based on polygon count, unlike subdivision surface.
