If I had blood/brutal imagery being perm-disabled for Under-13 year-olds, would it count as Mild due to it being disabled for less-mature ages, or would it be Moderate due to still being in the game / having realistic blood still in it, even though it would be disabled for Under-13 year olds?

It would be considered mild as it is not being displayed in the game. You can even put unspeakable things in your game but as long as they are not displayed to the audience it won’t count in the questionnaire.
Yeah but, it’s technically still being displayed to players who are over 13, whilst having it disabled for people under 13… meaning that it’s still in the game, but not displayed for people who aren’t 13+.
Hmm… You could mark it moderate and just mention that blood intensity would increase depending on the age?
That’s probably what I would do just to stay safe.
