I really like it. Seems a little dark though, maybe brighten it up.
This wall is looking pretty empty. I’m not an expert with hospitals but, there’s typically more machines in the room. A nightstand could spice up the room too, if the patient is staying overnight.

His left arm (or whatever) is bent weird, and is really hard to see. I don’t think thats how people lay in hospital beds

Using odd realism in this reply, it doesn’t look like nothing serious is with the patient, they just look sad and aren’t showing signs of any present illnesses. IV fluids aren’t really necessary, they don’t need any fluids replenished unless they’re dehydrated or have hypoglycemia or hyponatremia, although hypoglycemia would be solved with more sugary water and hyponatremia solved with saltier water, possibly 2%.
Usually, when a patient is admitted into a hospital they would do a series of tests to make sure they don’t have any issues by doing blood tests and stuff, but it looks like they are just letting the patient rest. The nurse also looks like they are about to move the patient to another room, which doesn’t usually happen, it’s done with more than one nurse.
The heart rate monitor isn’t connected to the patient, so I guess it’s just sitting there for now.
In terms of the render itself, the shadows aren’t too good, it looks like the doctor is floating because there aren’t any contact shadows. The nurse looks to be very reflective, and it’s the same to the three characters in this render. Roughness should be turned up.
The floor texture isn’t that bad, but it doesn’t match well with the rest of the room. The back wall has a texture while the other walls do not, which doesn’t make too much sense. There should be a texture with every brick in the render, otherwise there shouldn’t be any textures at all, excluding clothing and faces for the characters.
Overall I guess it’s pretty decent for a quick 30 minute image. Doesn’t look great to something that has more effort in it.
hate to burst your bubble but most of the people that are going to be seeing this are kids up to the age of 16 and most of them don’t know how hospitals work so the realism of the treatments and tests doesn’t matter.
